Basculación cecal: causa infrecuente de obstrucción intestinal.

Resumen

La báscula cecal es un tipo infrecuente de vólvulo de ciego que provoca obstrucción intestinal. Su etiología es multifactorial, donde el ciego se pliega en dirección ascendente sobre el colon derecho. Su diagnóstico debe sospecharse siempre que exista dilatación cecal ectópica. La TC es la prueba de imagen de elección, especialmente en pacientes graves.

Se presenta el caso de un hombre de 94 años que acude a Urgencias por cuadro séptico consistente en dolor abdominal generalizado asociado a leucocitosis de casi 20.000/mm³, a la que se le realizó una Tomografía Computarizada con diagnóstico final de obstrucción intestinal por basculación anterior del ciego.

Abstract

Cecal bascule is a rare type of cecal volvulus that causes intestinal obstruction. Its etiology is multifactorial, where the cecum folds upwards on the right colon. Its diagnosis should be suspected whenever there is ectopic cecal dilatation. CT is the imaging test of choice, especially in seriously ill patients.

We present the case of a 94-year-old man who came to the Emergency Department due to septic symptoms consisting of generalized abdominal pain associated with leukocytosis of almost 20,000/mm³, who underwent a Computed Tomography with a final diagnosis of intestinal obstruction due to anterior tilting of the blind.
